Chronic Musculoskeletal Pain: A Population Health Management Approach

People with chronic musculoskeletal pain, including fibromyalgia need personalised care – an approach offering patients choice and control over their mental and physical health, basing care on what matters to them personally, and focusing on individual strengths and needs.
This session describes a population health model offering people with chronic pain personalised support in primary care, with MDT support for the primary-care team from specialists including Rheumatology, Physiotherapy and Health Psychology.
Learning outcomes:
- Be able to explain the core principles of population health management for people with chronic pain.
- Be able to list the priorities of people with chronic pain and the services they may choose when offered personalised care.
- Be able to describe how to deliver a PCN-based model of care for people with chronic pain can work, supported by a specialist MDT.
