This session explores how to deliver the 8 Key Care Processes (8KCP) for diabetes in line with the 2026/27 updated QOF requirements. Drawing on insights from his PCN DIAST Plus project work, Dr Ray will share pragmatic strategies to improve completion rates, optimise recall systems, and reduce unwarranted variation in care. Delegates will gain clear, actionable insights into embedding the 8KCP within routine primary care, strengthening coding and monitoring processes, and addressing health inequalities to improve outcomes for people with type 2 diabetes.
