This talk will cover the present landscape of palliative and end of life care in the UK, the current challenges that primary care faces and the offer of support to attain the vision of a seamless Gold Standard of Care for all at the end of life, available from the new and updated Gold Standard Framework training and accreditation programme for Primary Care. You will learn about End-of-Life care in numbers, understand the bottlenecks of care and how tailored, person centred care has been shown to have positive impact on patient outcomes, work force stress management, reduction in crises- including a reduction in avoidable admissions and for the health economy. This talk is for ICS leads in palliative care, PCN CDs and any GPs, trainees, Nurses, Pharmacists, ARRS roles involved in care of the elderly, care home care or with an interest in end of life care.
