This session will consider the role of the pharmacy team in the prevention and treatment of cardiovascular disease in primary care.
Learning Outcomes:
1) Give an overview of the pathways/guidelines for the management of CVD in primary care
2) Consider the role for the pharmacy team in the implementing the pathways/guidelines in practice
3) Reflect on own workplace and how might take forward the management of CVD in everyday practice.
